MY PERSONAL DETAILS?
It is important that you let us know of
any change of name, address or telephone number, in order to enable your records
to be kept up to date. If you leave the area it will be necessary to register
with another doctor. You do this by completing the relevant section of your medical
card and taking it to the new surgery of your choice.
