It is important that you let us know of any change of name, address or telephone number, in order that your records are kept up to date. You may download the relevant forms from the download forms tab. If you leave the area it will be necessary to register with another doctor. You do this by completing the relevant section of your medical card and taking it to the new surgery of your choice.
